If you are a new patient to our clinic, please arrive at least five minutes before your consultation or fill out a new patient form here and bring the following to your consultation:

  • Photo ID
  • A valid Medicare card
  • Workcover patients will need to inform their employer and bring their employer’s contact details for verification to arrange payment.

Fees

Our Fees

We are a bulk billing practice to patients holding a valid Medicare card. Some services are excluded and an out of pocket expense will apply. Fees will vary depending on the service.

The following fees apply to patients who do not hold a Medicare card or OSHC/OVC membership, the following fees do apply;

  • Short consultation — $50
  • Standard consultation — $75
  • Long consultation — $120
  • Prolonged consultation — $150

Home visits and additional services will attract out of pocket fees. The fees may vary depending on the complexity of the service and procedure required.

Privacy

We take the privacy and security of your information seriously. Our clinic is equipped to protect the confidentiality and privacy of your health records.

The Practice acknowledges patients may request access to their medical records. Patients are encouraged to make this request in writing, and the Practice will respond within a reasonable time. The Practice will take reasonable steps to correct personal information where it is satisfied they are not accurate or up to date. From time to time, the Practice will ask patients to verify the personal information held by the Practice is correct and up to date. Patients may also request the Practice corrects or updates their information, and patients should make such requests in writing.

We have been accredited to the standards required by the Royal Australian College of General Practitioners (RACGP).

New Patients Welcome

Our practice is appointment based, however we always welcome and accommodate walk-in patients. Our standard consultations are up to 10 minutes.

Practice Policies

After-hours Consultations

Aya Family Healthcare After Hours provides access to bulk billed in-home after-hours medical care for patients who cannot contact their regular GP, please contact (02) 8215 0083.

Results

For their test results, all patients are encouraged to see their normal GP (since the treating doctor and other doctors at AMDS do not provide continuity of care) (as all request forms are copied to the patients regular GP). If the results are abnormal or life-threatening, our clinical staff will contact the patient to verify if they have seen their regular GP, and if not, they will be sent to see their regular GP or any other GP in AMDS to discuss the results.

Correspondence

Electronic medical information is transmitted via encrypted format using secure messaging software. Printed medical information is mailed through the secure post (i.e., with a tracking number). Patient diagnostic correspondence and results, whether in electronic or hard copy format, are always forwarded to the relevant doctor. The word “Confidential” is written on the correspondence. Patient information is only sent via email if it is securely encrypted. We do not accept patient enquiries or requests via email. We also do not send patient information to personal email addresses.

Communication Assistance

Information: Our GPs do not take phone calls during consultation hours. Staff will take a message and pass message to GP to return calls. We do not communication with patients via email.

Complaints

All suggestions and complaints are promptly addressed. Every member of our practice team is trained to deal with complaints as they arise.
If the matter cannot be resolved, the patient is advised about how to contact the external health complaints agency in New South Wales.

What is MyMedicare?

The Australian Government has introduced a new voluntary patient registration model called MyMedicare to drive improvements in primary health care for all Australians and deliver new funding packages to primary care providers. MyMedicare will strengthen the relationship between patients and their health care teams by allowing patients to register with their regular general practice and select their preferred GP and providers within the practice. What are the benefits of MyMedicare for our patients?

The key benefits to you include better access, better continuity of care, and better planning. Registration in MyMedicare is not compulsory, however it is recommended to improve continuity of care for patients. You can also change your mind and leave the program at any time. It does not affect your Medicare rights or benefits. Please reach out to our team if you have any questions.
